Team Leader – Primary Care Project

£27,741 per annum (Paid)
37 hours per week
Contract term: 9 months fixed, with possibility of extension
This role will lead our Primary Care Project planning, delivering and evaluating project activity. You will ensure delivery of quality services to carers, working closely with NHS, Social Care and voluntary sector partners to identify carers, obtain referrals and inform good practice in relation to carers.
The successful applicant will be a skilled operational manager with excellent communication, IT and organisational skills. Experience of addressing barriers to inclusion and enabling seldom heard communities to access services is essential. You will also bring experience of delivering group and one-to-one support to carers, or other vulnerable clients with complex needs.
The post holder will work across a range of community settings, with office space usually being provided at Care for the Carers’ office in Eastbourne (currently closed due to Covid-19), with remote working available. The post holder’s main community duties will usually take place within the Hastings area, within GP practice settings, however, work anywhere within East Sussex may be required.
